Hallmark Health hospitalists offer additional level of care

At Hallmark Health’s Lawrence Memorial Hospital of Medford and Melrose-Wakefield Hospital patients are offered this additional level of care through our hospitalists. This dedicated group is made up of physicians board-certified in internal medicine or family medicine that specialize in caring for patients in the hospital setting. Hospitalists do not have outpatient medical practices and are available to patients throughout the day and are most familiar with the needs of hospitalized patients.

Most hospitals and many primary care physicians throughout the country have adopted the hospitalist model. Hospitalists know every specialist and department in the hospital and work very closely with them to make sure patients’ care is well coordinated. As hospitalists focus on inpatient medical care, follow up of test results is very quick and adjustment of treatment regimens occurs based on those test results throughout the day. Communication with patients’ primary care physicians occurs via telephone and/or email at the time of admission, during patients’ hospital stay and at the time of discharge. Following discharge, patients return to the care of their primary care physician.

Hallmark Health System employs approximately 20 hospitalists that work at both Lawrence Memorial Hospital of Medford and Melrose-Wakefield Hospital.
