To donate to the Tribute Board click here or download this form and complete it and send check payable to Hallmark Health to the address provided on the form. Hats On will feature a Tribute Board at the event.  The Tribute Board honors those whose lives have been touched by cancer as well as provide an opportunity to pay tribute to a caregiver that has made a difference in your life or the life of someone you love. Donations in any amount are accepted and can be made on-line or can be called in with a credit card to 781-979-6116.

The Hallmark Health Cancer Center offers patients state-of-the-art, comprehensive cancer programs provided by askilled, multidisciplinary team of professionals. The Cancer Center, comprised of The CHEM Center for Radiation Oncology and The Hematology Oncology Center, offers a continuum of cancer care from prevention and early detection to diagnosis and treatment.

Funds raised from Hats On will support The Cancer Center and bring us closer to realizing our goal of opening a boutique in our facility that allows those undergoing treatment to be fitted for wigs, scarves, hats and prosthetic devices in a convenient, dignified and caring atmosphere. Your participation and support of this special event will also assist Hallmark Health in fulfilling its mission to provide high quality, state-of-the-art and compassionate cancer care to our patients.
