What if I am sick and need to be seen today?
We have same day sick appointments if you are sick and need to be seen today. Please call the office and we will do our best to fit you into the physician's schedule. If the office is extremely busy, we may request that you be seen at the Lawrence Memorial Hospital Prompt Care center.
Forms
The physicians at the Hallmark Health Medical Associates -
Work or school notes will be given to the patient at the time of the visit. School and work notes will not be issued if the patient has not been seen.

School physical forms will be completed in 24-48 hours.

Work physical forms will be completed in 24-48 hours.

DOT physical forms will be completed in 24-48 hours.

Any legal forms will be completed in one week.

Disability forms will not be filled out for new patient visits. Patients must be established within the practice for at least three-six months.

Prior authorizations for medications will be filled out in 24-48 hours.

Prior authorizations for procedures will be filled out within 24-48 hours of procedure.

Disability forms for established patients will be completed within one week.

Forms that require just a signature must be left off and the physician will complete the form. Please do not expect to wait for the form to be signed. Forms must be left and patients will be called within 48-72 hours when form is completed.

Services
Adult Care:
We offer comprehensive primary care services for all adults including annual physical exams, preventive health screening, acute visits, and chronic disease management. We also specialize in preventative health care and can assist with smoking cessation, weight loss, nutritional counseling, and stress management.
Newborn & Child Care:
As family physicians we are trained to provide comprehensive newborn, child, and adolescent care. Whether your child is a few hours old or a teenager we can provide routine examinations, immunizations, and developmental screening. We also encourage you to bring your child for non-emergency acute care including mild asthma attacks, cuts that may require stitches, and sprains.
Occupational Health:
We are happy to see you for pre-employment physicals, immunizations, and Department of Transportation (DOT) examinations.
Women's Health:
Women regardless of their age have special health care needs. Our practice is trained to provide a wide range of services including complete gynecologic exams, breast exams, pap tests, endometrial biopsies and STD testing. We also provide counseling for premenstrual syndrome and domestic violence and treatment for osteoporosis and menopause.
Procedures
Equipped with state of the art procedure rooms, we are able to perform endometrial biopsies, IUD insertion and removals, Implanon insertion and removals, skin biopsies and mole removals in the Health Center. We routinely stabilize injured joints and repair minor cuts and lacerations.
Emergency and After Hours Calls
Our Family Physicians are available for emergency questions 24 hours a day, seven days a week by simply calling the office phone number 781-338-7400. If you have an after hours question or concern that is not an emergency, such as a prescription refill or chronic medical issue, please call the next morning during regular office hours.
