At Hallmark Health, we provide pulmonary rehabilitation to individuals who have lung diseases including: COPD; Emphysema; Chronic Bronchitis; Pulmonary Fibrosis: Dyspnea; Asthma; Post-op lung surgery and Bronchiectasis. Other lung diseases may quality for treatment. The Pulmonary Rehabilitation Team has designed, according to national guidelines, a comprehensive program from evaluation to exit to help patient maintain a healthier lifestyle and better manager their disease.
Our approach
Our rehabilitation program consists of two phases. The first phase is a closely-monitored exercise class where you are supervised by a registered nurse, physical therapist and an exercise physiologist. Weekly education topics are incorporated into the sessions. The second phase is a supervised exercise group with one or two sessions per week. This is a voluntary program.
Eligibility
To join the pulmonary rehabilitation program you must be referred by your primary care doctor or pulmonologist. Research has shown that patients in a pulmonary rehabilitation program including exercise and education maximize their ability to manage daily activities and improve their quality of life.

Our team is dedicated to optimizing your level of activity and function in a safe environment. Our team members continually monitor your progress and adapt your exercise program as you progress. Monthly activity and educational progress updates are sent to your physician.
Medical insurance
Health insurance coverage varies. Some insurance programs require prior approval before enrolling in the program. Please consult your insurance company about your coverage prior to your first visit with us. The maintenance program is a voluntary program which is self-pay and is not covered by any insurance carrier.

Hours of operation
We are available Tuesday and Thursday from 8:30 a.m. to 4:30 p.m. We make every effort to accommodate patient’s scheduling needs.
