My Patient Portals is your online access to your patient care information and connection with your physicians' offices.

Hallmark Health offers two levels of patient portals to help you better manage information related to your healthcare services. You may need to access both portals, depending on where you receive your care.

For issues accessing either Patient Portal, please call 781-338-7200, Monday through Friday, 8 a.m. to 4 p.m.

Doctor's Office Patient Portal 

Do you want to connect with your physician’s office to view or send a secure message to your provider? If so, access this secure site to send an email, request an office appointment, prescription refill, or view your health information including allergies, advanced directives, health conditions, medication list, immunizations and routine lab results.  

The Physician Office Patient Portal will include information related to visits with your Hallmark Health primary care physician and/or specialist including OB/GYN, cardiologist, endocrinologist, surgeon, and other specialties.

How do I sign up?

Visit your physician’s office to receive your unique PIN. Once you have received your PIN, complete the registration steps to access the site. Click here; then register.

Hospital Patient Portal

If you received care at Lawrence Memorial Hospital of Medford, Melrose-Wakefield Hospital, Health Image, Hallmark Health Medical Center, Reading or The CHEM Center for MRI, the Hospital-based Patient Portal provides secure access to your electronic medical record.

View your laboratory results, imaging results (X-ray, CT, MRI), medications, allergies, medical problems, discharge instructions, visits, outpatient appointments and more.

How do I sign up?

If you have been given a Personal Identification Number (PIN) during your hospital visit, Click here, then sign up

If you are a first-time user, you need a secure PIN. You can get your secure PIN by visiting the Release of Information Department within Medical Records at either hospital campus. A valid picture I.D. is necessary. The Release of Information Department at Melrose-Wakefield Hospital is open Monday through Friday, 8 a.m. - 4 p.m. and the Release of Information Department at Lawrence Memorial Hospital of Medford is open Monday, Wednesday and Friday from 8 a.m. - 2 p.m.
